H5Fis_accessible() created a new file handle and attempted to read
through it, which will fail when a file has been opened with an
exclusive lock on operating systems that have mandatory locks.

This change uses the same scheme we use in H5Fopen() to check if
the file is already open and only tries to read the file signature
if the file has not already been opened.

Also adds a test for this behavior.

For error_test.c the removed code was already dead, because it was in the else of an `#if H5_USE_16_API` block.

Based on H5Location.h, I think p_get_ref_obj_type was meant to be in `#ifndef DOXYGEN_SHOULD_SKIP_THIS` and an `#endif` was missing.  Similarly, in the header, getObjTypeByIdx is only in H5_NO_DEPRECATED_SYMBOLS, not DOXYGEN_SHOULD_SKIP_THIS.

Clean up many mismatches between malloc/free and H5MM_malloc/H5MM_xfree
in the library and tests (and use of H5free_memory and H5Dvlen_reclaim).  Also
make H5Ocopy use a private version of H5Lexists, which doesn't internally
throw (and suppress) errors when an object (or the path to it) isn't found in
the destination.

1023374492 [svn-r27768] Description:
Complete revamp of package initialization/shutdown mechanism in the library.
Each package now has a single init/term routine.

    This new way should avoid packages being re-initialized during library
shutdown and is also be _much_ more proactive about giving feedback for
resource leaks internal to the library.

    Introduces a new "module" header file for packages in the library
(e.g src/H5Fmodule.h) which sets up some necessary package configuration macros
for the FUNC_ENTER/LEAVE macros.  (The VFL drivers have their own slightly
modified version of this header, src/H5FDdrvr_module.h)

    Also cleaned up a bunch of resources leaks all across the library and tests,
along with addressing many warnings, as I encountered them.

Tested on:
    MacOSX/64 10.10.5 (amazon) w/serial & parallel
    Linux/64 3.10.x (kituo) w/serial & parallel
    Linux/64 2.6.x (ostrich) w/serial

2e6100fb23 [svn-r27572] Removed VMS-specific code from the library.
The only remaining code consists of a few floating-point tests
that rely on pre-generated and checked-in VMS files. These have
been left alone, even though they will not be possible to
recreate, since testing VMS float behavior is still important.

d01ee66666 [svn-r24937] Adds an H5free_memory() function to the library for use with
functions that return library-allocated memory. The test and tool
code were modified to use this new function where it's appropriate.

e6cace9ee4 [svn-r24376] Updated all code to use HDmalloc, HDcalloc, and HDfree.
Prep work for adding a memory free function, part of HDFFV-8551.

Tested on:
    32-bit LE linux (jam) 
        * with C++, Fortran
        * with thread safety on

    These are minor changes (almost entirely renaming foo functions
    to HDfoo) that don't change functionality.  This should not
    require a full commit test.

Maintenance: Fixed the bug found by coverity CID 788
There were two problems with this function:
    1) it tried to unnecessary free NULL pointer
    2) it tried to allocate c_name buffer that is done by H5Pget_class_name

6e0f6e4ebc [svn-r21736] Issue 7582 - The library allowed the conversion of strings between ASCII and UTF8. I added more test cases to the previous commit. Now it has conversion from UTF8 to ASCII, ASCII to UTF8, VL and fixed length, and H5Tconvert.

68da0f7736 [svn-r21734] Issue 7582 - The library allowed the conversion of strings between ASCII and UTF8. I corrected it by adding a condition check in H5T_conv_s_s and H5T_conv_vlen to report an error under this situation.

157623de6a [svn-r20913] Issue 7579 - The overflowing ENUM values are inconsistent. When no conversion is involved in reading or writing the
data, overflowing values are retained.  When conversion happens, the values become -1. The conversion function puts -1
when overflow happens.  I added two new dataset transfer property to control whether to fill 0xff in the destination 
data or convert to the destination data when overflow happens.  The two new functions are H5Pset(get)_enum_conv_overflow.  I also added test cases in enum.c and dtypes.c.

Tested on jam, koala, and heiwa.

Correct several problems with compound datatypes that don't have any
fields added:

- Change assertion to error report when a file is encountered which has this
	situation.

- Added check to attribute creation to avoid creating attributes with a
	datatype like this (datasets and named datatypes already have the check)

4f75c8c24b [svn-r20121] Bug 1586 - the datatype handler created with H5Tencode/decode had the reference count 0. I fixed it by changing the APP_REF parameter of H5I_register from FALSE to TRUE and added a test case in dtypes.c.
Tested on jam, heiwa, and amani.
